เอกสารที่ต้องใช้ในการนําเข้า MOU

เอกสารนายจ้าง

  • สำเนาหนังสือรับรองบริษัท อายุไม่เกิน 6 เดือน (ใช้ในกรณีที่นายจ้างเป็นนิติบุคคล) ลงนามพร้อมประทับตราสำคัญบริษัท จำนวน 5 ชุด
  • สำเนาบัตรประชาชน และ สำเนาทะเบียนบ้าน ของนายจ้าง อย่างละ 5 ฉบับ
  • แผนที่สถานประกอบการ ลงนามพร้อมประทับตราสำคัญบริษัท จำนวน 1 ชุด
  • รูปถ่ายกิจการ และป้ายสำนักงาน จำนวน 1 ชุด

กรณีเป็นกิจการรับเหมาก่อสร้าง ต้องเพิ่มเอกสารดังนี้

  • สำเนาสัญญารับเหมาก่อสร้าง พร้อมประทับตรา บริษัทคู่สัญญา จำนวน 1 ชุด
  • สำเนาหนังสือรับรองบริษัทของบริษัทคู่สัญญา อายุไม่เกิน 6 เดือน ลงนามพร้อมประทับตราสำคัญบริษัท จำนวน 1 ชุด
  • สำเนาบัตรประชาชน และ
  • สำเนาทะเบียนบ้าน ของกรรมการในบริษัทคู่สัญญา อย่างละ 1 ฉบับ

 

เอกสารแรงงาน (สำหรับ MOU Return)

  • สำเนาหน้าพาสปอร์ต 1 ฉบับ
  • รูปถ่าย 1 นิ้ว 3 รูป
